申请代办使馆加签注意事项：

　　1、要提供清洁合格的加签文件，避免用不清洁或破损的文件，避免使用过期的原产地证，出具的ci要正规，ci纸太薄的要粘贴一张白纸。

　　2、加签文件须有公司章和贸促会加签章(及签名)，避免印章漏盖、反盖或多盖以及印章不清，文件附页应粘连在正页上．并在交接处加盖贸促会骑缝章。

　　3、由商检局出具的文件上不能再加盖贸促会加签章，否则将视为无效。

　　4、要详填"代办申请使馆加签申请表"、中各栏目(有固定格式，与我索取)，尤其是"申请单位、地址、邮编、te、传真及经办人"、"使用国别"、"送往国别"、"送往使馆"及"文件名称、份数及编号"也需填写清楚。文件份数控页计算，如需加签产地证一正二付，应写3份。"返回时间"可署具体日期或"尽快"字样。一个单位申请送办二个以上使馆，每个使馆写一张申请表。

　　5、产地证中第l栏与1栏的公司名称必须相符，ci上公司名称与所盖公司也需相符。

　　6、产地证中第6-10栏的内容如有错不能更改、只能重新制单。

　　7、ci的单价、数量与总金额的计算不能有错。

　　8、产地证签发日期不能早于ci日期。

notes for applying for the endorsement of the embassy:
